GLP-1 medications like semaglutide and tirzepatide have the strongest evidence of anything in the weight-management space — and, like any effective medication, they come with side effects and an important safety warning. Here’s the honest, plain-language rundown.

The common side effects

Most GLP-1 side effects are gastrointestinal:

  • Nausea (the most talked-about)
  • Reduced appetite (partly the point, but it can go too far)
  • Constipation or diarrhea
  • Occasional reflux or stomach discomfort

These tend to be most noticeable early on and each time the dose steps up, and for most people they ease as the body adjusts.

The serious warning you should know

This is the part that matters most: GLP-1 and dual-agonist medications carry an FDA Boxed Warning regarding thyroid C-cell tumors and are not appropriate for everyone. A provider screens your personal and family history — including medullary thyroid cancer and MEN 2 — and reviews pancreatitis, gallbladder, and severe GI history before prescribing. This is exactly why these aren’t sold from a menu.

How side effects are actually managed

The single biggest lever is slow dose titration — starting low and stepping up gradually so your body adapts. Beyond that:

  • Eating smaller, lower-fat meals and staying hydrated
  • Not rushing the dose just to speed results

What “manageable” doesn’t mean
